BASIC RECREATIONAL / SPORT DIVER CREDENTIAL (RENEWAL)
| 1. | Identity Card. |
| 2. | High-resolution, sharp photograph, blue background, formal attire (both shoulders and face visible, updated within the last six months, neutral facial expression, no glasses, no groomed patterns in the beard for men, no low-cut necklines for women, no photomontages). Must be attached in the SEAFARER’S FILE section. |
| 3. | Expired document (Basic Recreational / Sport Diver Credential, both sides). It must be attached in the SEAFARER’S FILE section, under the CERTIFICATES tab. |
| 4. | Record of refresher training (if the person has not dived in 6 months). |
| 5. | Diver’s Medical Report (must be issued by an INEA-authorized Physician or an Internist from the Public or Private Network). |
| 6. | Payment for document issuance, to the Banesco Bank Checking Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS’ PROCEDURE PAYMENT TABLE according to Article 61, Item 20 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the request and is valid for three months once it is made”. |
| 7. | INEA payment liquidation form will be uploaded to each user in their “SEAFARER’S FILE”, under the “REMARKS” tab by the Seafarers Management personnel once their payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
