FIRST-CLASS MOTORMAN TITLE CREDENTIAL (COPY)
| 1. | Identity Card. |
| 2. | Updated photograph with good resolution and clarity, red background, Uniform No. 2 without kepi (shoulder boards with one 5-millimeter stripe), or formal attire (both shoulders and face visible, no glasses, no facial expressions, no designs in the beard for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE (EXPEDIENTE DE MARINO) section. |
| 3. | In the event of damage, deterioration, loss, theft, or robbery, if the user does not have a record of the document in the SINEA, a PDF copy of the First-Class Motorman Title Credential with a manual signature must be scanned, if available. Otherwise, a report filed with the competent judicial authority or an explanatory letter detailing the events and providing the information of the requested document must be uploaded.. |
| 4. | Payment for the concept of document issuance, to the Banesco Bank checking account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to Article 61, Item 21 (located next to the TYPE OF REQUEST). This Table will be updated to the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and is valid for three months once it is completed.” |
| 5. | The INEA payment liquidation form will be uploaded to each user’s SEAFARER’S FILE, under the OBSERVATIONS tab, by the Seafarer Management personnel once the payment has been confirmed. |
Note: All documents must be valid, legible, digitized in PDF format, and attached to the SINEA.
