FORM P (COPY)
| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, blue background (deck/navigation) or red background (engine) according to specialty, with Uniform No. 2 and visible stripes if the applicant is a cadet, or formal attire if a trainee (with both shoulders and face visible, updated within the last six months, with a neutral facial expression, no styled or patterned beard for men, and no low-cut necklines for women, nor photomontages). This must be attached in the SEAFARER FILE section. |
| 3. | In the case of a copy due to loss, theft, or robbery, a copy of Form Q1, Q2, or P, if held, must be scanned in PDF format; otherwise, a report filed with the competent judicial authority must be uploaded. In the case of a copy due to damage or deterioration, the original must be scanned and submitted (upon disembarkation) for invalidation. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
