CERTIFICATE OF ENDORSEMENT OF RECOGNITION FOR SECOND MATE CERTIFICATE (COPY)

1. Identity Card.
2. High-resolution and sharp photograph, blue background, in uniform (rank must be visible on both shoulders, updated within the last six months, neutral facial expression, no glasses, no groomed patterns/shapes in the beard for men, and no low-cut necklines for women, no photomontages). This must be attached in the SEAFARER FILE section.
3. In the case of a copy due to loss, theft, or robbery, if the user does not have a record of the document in the SINEA system, a PDF copy of the Certificate of Endorsement of Recognition for Second Mate Certificate with a handwritten signature must be scanned if available; otherwise, a formal complaint filed before the competent judicial authority must be uploaded. In the case of a copy due to damage or deterioration, the affected Endorsement or an explanatory letter of the events occurred must be scanned and submitted.
4. Credential (both sides) or Second Mate Certificate issued by the Maritime Administration of the applicant’s nationality and duly verifiable through available means. This must be attached in the SEAFARER FILE section, under the CERTIFICATES tab.
5. Payment for the issuance of the document, made to the Banesco Current Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS PROCEDURES PAYMENT TABLE pursuant to Article 59, Item 12 (located next to the TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the request and is valid for three months once it is made”.
6. The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE,” under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
