CHIEF ENGINEER OFFICER CERTIFICATE ENDORSEMENT (COPY)
| 1. | Identity Card. |
| 2. | High-resolution, sharp photograph with a red background, wearing Uniform No. 2 (epaulettes with four 14 mm stripes, both shoulders and face visible, updated within the last six months, neutral facial expression, no groomed patterns in the beard for men, no low-cut necklines for women, and no photomontages). This must be attached to the SEAFARER FILE section. |
| 3. | In the event of a copy due to loss or theft, if the user does not have a record of the document in SINEA, a PDF copy of the Chief Engineer Officer Certificate Endorsement with a manual signature must be scanned if available; otherwise, a formal complaint filed with the competent judicial authority must be uploaded. In the case of a copy due to damage or deterioration, the affected Endorsement or an explanatory letter of the events must be scanned and submitted. . |
| 4. | Credential (both sides) or Chief Engineer Certificate. This must be attached to the SEAFARER FILE section, under the CERTIFICATES tab. |
| 5. | Payment for document issuance, to the Banesco Current Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to Article 59, Item 12 (located next to the APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and remains valid for three months once processed”. |
| 6. | INEA payment settlement form will be uploaded to each user in their “SEAFARER’S FILE”, under the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
