ENDORSEMENT OF RECOGNITION OF THE TITLE OF FIRST MATE (COPY)
- Identity Card.
- High-resolution and sharp photograph, blue background, in uniform (rank must be visible on both shoulders, updated within the last six months, neutral facial expression, no glasses, no groomed patterns/shapes in the beard for men, and no low-cut necklines for women, no photomontages). This must be attached in the SEAFARER FILE section.
- In the case of a copy due to loss, theft or robbery, if the user does not have a record of the document in the SINEA, a copy of the Endorsement of Recognition of the Title of First Officer of Navigation with manual signature must be scanned in PDF format if the user has it; otherwise, a complaint must be filed before the competent judicial authority. If it is a copy due to damage or deterioration, scan and submit the affected endorsement or letter explaining the events that occurred.
- Credential (both sides) or Chief Mate Certificate issued by the Maritime Administration of the applicant’s nationality and duly verifiable by available means. This must be attached in the SEAFARER FILE section, under the CERTIFICATES tab.
- Payment for document issuance fees, to the Banesco Checking Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS PROCEDURE PAYMENT SCHEDULE pursuant to Article 59, Item 12 (located next to the APPLICATION TYPE). This Schedule will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the application and has a three-month validity period once processed”.
- The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE,” under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
