ENDORSEMENT OF THE TITLE OF FISHING MACHINERY CHIEF (COPY)
| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, red background, formal attire (both shoulders and face visible, no more than six months old, neutral facial expression, no designs in facial hair for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section. |
| 3. | In the case of a copy due to loss, theft or robbery, if the user does not have a record of the document in the SINEA, a copy of the endorsement of the title of Chief of Fishing Machinery with manual signature must be scanned in PDF format if the user has it; otherwise, a complaint must be filed with the competent judicial authority. If it is a copy due to damage or deterioration, scan and consign the affected endorsement or letter explaining the events that occurred. |
| 4. | Credential (both sides) or Fishing Chief Engineer Certificate of Competency. This must be attached in the SEAFARER FILE section, under the CERTIFICATES tab. |
| 5. | Payment for document issuance, to the Banesco Current Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to Article 59, Item 12 (located next to the APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and remains valid for three months once processed”. |
| 6. | INEA payment settlement form will be uploaded to each user in their “SEAFARER’S FILE”, under the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
