ENDORSEMENT OF FISHING CAPTAIN’S CERTIFICATE (COPY)
| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, with a neutral facial expression, without patterns in the beard for men, and without low-cut necklines for women, nor photo montages). It must be attached in the SEAFARER FILE section. |
| 3. | In the case of a copy due to loss, theft or robbery, if the user has no record of the document in the SINEA, a copy of the Fishing Captain’s Title Endorsement must be scanned in PDF format with manual signature if the user has it, otherwise a complaint must be filed with the competent judicial authority. If it is a copy due to damage or deterioration, scan and submit the affected Endorsement or a letter explaining the events that occurred. |
| 4 | .Credential (both sides) or Fishing Skipper Certificate. Must be attached in the SEAFARER FILE section, under the CERTIFICATES tab. |
| 5. | Payment for document issuance, to the Banesco Current Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to Article 59, Item 12 (located next to the APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and remains valid for three months once processed”. |
| 6. | INEA payment settlement form will be uploaded to each user in their “SEAFARER’S FILE”, under the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
