    ORIGINAL EXTERNAL ISM CODE AUDITOR
    EXTERNAL ISM CODE AUDITOR (ORIGINAL)
    1. Identity Card.
    2. Updated curriculum vitae (CV) summary, including certificates accrediting:
      • Two (02) years of shipboard experience as a Merchant Marine Officer or an Officer of the Bolivarian National Navy, and having conducted a minimum of five (05) company audits and five (05) ship audits alongside a duly certified ISM Auditor; or
      • Bachelor’s Degree in Maritime Transportation or Naval Engineer, serving within the National Institute of Aquatic Spaces (INEA) in the area of application of the ISM Code, and having conducted a minimum of five (05) company audits and five (05) ship audits alongside a duly certified ISM Auditor.
    3. High-resolution and sharp photograph, blue background, Uniform No. 2 (visible stripes according to hierarchical rank) or blue background, formal suit (both shoulders and face visible, updated no more than six months ago, with a neutral facial expression, without glasses, without patterns in the beard for men, and without plunging necklines for women, nor photomontages). It must be attached in the SEAFARER FILE section.
    4. Merchant Marine Credential (both sides) or License. It must be attached in the SEAFARER’S FILE (EXPEDIENTE DE MARINO) section, under the LICENSES/TITLES (TÍTULOS) tab (Optional).
    5. Designations or agreements between the parties for five (05) company audits and five (05) ship audits under the ISM Code with a duly certified ISM Auditor.
    6. University Degree.
    7. The IMO Courses (required and additional, both sides, including registration and folio numbers). The validity of the requested document depends on the date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab.
    8. Maritime Medical Certificate (physical and mental fitness with vision and hearing specification), in the Maritime Medicine format found on the INEA website (must be performed by a Physician authorized by the INEA or an Internist Physician from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).
    9. Payment for document issuance, to the Banesco Bank Checking Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE pursuant to Article 61, Item 16 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the request and is valid for three months once completed”.
    10. The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE”, under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.

    Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
