ORIGINAL-ISSUANCE OF PERSONAL SEAFARER’S CERTIFICATE HOLDER

PERSONAL SEAFARER’S CERTIFICATE (ORIGINAL-ISSUE)
| 1. | Identity Card. |
| 2. | Updated, high-resolution and sharp photograph, blue background (deck/navigation) or red background (engineering), Uniform No. 2 with or without a combination cap/kepi (shoulder boards with stripes visible according to hierarchical rank) or formal suit; for Fishing Officers, blue background (deck/navigation) or red background (engineering) formal suit, (both shoulders and face visible, no glasses, neutral facial expression, no groomed patterns/shapes in the beard for men, and no low-cut necklines for women, no photomontages). This must be attached in the SEAFARER FILE section. |
| 3. | Certificate Credential (both sides) or Merchant Marine Certificate. This must be attached in the SEAFARER FILE section, under the CERTIFICATES tab. |
| 4. | Form Q-1, Q-2 or P (minimum 12 months sailed) or Support Personnel Seaman’s Certificate (all pages used). |
| 5. | Payment for the issuance of the document, to the Cte. Account of Banco Venezuela, in the name of INEA, 0102-0762-25-0000003188, RIF G-20002141-1, according to the amounts indicated in the SEA PEOPLE PROCEDURES PAYMENT TABLE according to Article 60, Item 1 (located next to the TYPE OF APPLICATION). This Table is updated at the DICOM rate every Monday at noon. *The date of the payment made must correspond with the date of the application and is valid for three months after it is made “*. |
| 6. | The INEA payment liquidation form will be uploaded to each user’s SEAFARER’S FILE, under the OBSERVATIONS tab, by the Seafarer Management personnel once the payment has been confirmed. |
Note: All documents must be valid, legible, digitized in PDF format, and attached to the SINEA.
Note: To collect the Seafarer Book at the Port Captaincies, users must print Form J and present it at the Port Captaincy where they submitted their application, attaching 2 passport-sized photographs with the same characteristics as those described in the requirements.
