OFFSHORE DRILLING UNIT CERTIFICATE (MODU CODE) (ORIGINAL)
- Identity Card.
- Updated Curriculum Vitae (CV)
- Passport (Optional).
- Photograph with good resolution and clarity, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, neutral facial expression, without glasses, no beard styling or patterns for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section.
- University Degree (Optional).
- The IMO Courses (required and additional, both sides, including registration and folio numbers). The validity of the requested document depends on the date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab.
- Employment letter or official communication issued by the contracting company, specifying the position to be held.
Maritime Medical Certificate (physical and mental fitness with vision and hearing specification), in the Maritime Medicine format found on the INEA website (must be performed by a Physician authorized by the INEA or an Internist Physician from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).- Payment for document issuance to the Banesco Checking Account, under the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE pursuant to Article 59, Item 5 (located next to APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and remains valid for three months once processed.”
- The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE”, under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
