RECREATIONAL / SPORT INSTRUCTOR ASSISTANT CREDENTIAL (ORIGINAL)
| 1. | Identity Card. |
| 2. | High-resolution, sharp photograph, blue background, formal attire (both shoulders and face visible, updated within the last six months, neutral facial expression, no glasses, no groomed patterns in the beard for men, no low-cut necklines for women, no photomontages). Must be attached in the SEAFARER’S FILE section. |
| 3. | International Certification or Advanced Recreational / Sport Diver Credential (both sides). |
| 4. | Affiliation Certificate from the Certifying Agency (PADI, CMAS, WRSTC, RSTC, among others). |
| 5. | Refresher training certificate (in case of not having engaged in diving activities for 6 months). |
| 6. | Diver’s Medical Report (must be issued by a physician authorized by INEA or an Internist from the Public or Private Network). |
| 7. | Payment for the issuance of the document, to the Banesco Bank Checking Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to Article 61, Item 20 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and is valid for three months once it is carried out”. |
| 8. | INEA payment liquidation forms will be uploaded for each user in their “SEAFARER’S FILE,” under the “REMARKS” tab by the Seafarer Management personnel once the payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
