    ORIGINAL SECOND-CLASS SPORT PATRON LICENSE

    GRAN MISIÓN TRANSPORTE VENEZUELA LETTERHEAD

    SECOND-CLASS SPORT PATRON LICENSE (ORIGINAL)

    1. Identity Card.
    2. Updated photograph with high resolution and sharpness, blue background, formal attire (both shoulders and face visible, no glasses, no facial expressions, no shapes in the beard for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER’S FILE section.
    3. Maritime Medical Certificate (physical and mental fitness with eye and ear examination specifications), in the Maritime Medicine format available on the INEA website (must be conducted by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must also submit a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).
    4. Certificate (both sides) and Approval Grade Report for the course (Second-Class Sport Patron Project), issued by CENAVE in a single PDF document; those wishing to take the online exam must attach a letter indicating whether they are presenting for the first time through the online exam process or for subject recovery (specify which ones).
    5. Payment for document issuance, to the Banco de Venezuela checking account, under the name of INEA, 0102-0762-25-0000003188, RIF G-20002141-1, according to the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE as per the following Articles: Original: Article 61, Item 13 and Online Exam Retake: Article 61, Item 15 (located next to the APPLICATION TYPE). This Table is updated at the DICOM rate every Monday at noon. “The payment date must correspond to the application date and is valid for three months once made”.
    6. The INEA payment liquidation form will be uploaded to each user’s SEAFARER’S FILE, under the OBSERVATIONS tab, by the Seafarer Management personnel once the payment has been confirmed.

    Note: Present the examinations corresponding to the license applied for, in accordance with the online testing procedure established in Circular No. 33 of 2021, published on the INEA website:
    https://www.inea.gob.ve/, under PROCEDURES – SEAFARERS – CIRCULARS.
    Note: All documents must be valid, legible, digitized in PDF format, and attached to the SINEA. You must request the License at the Port Authority where it will be used. Do not select Central Headquarters.
