Requirements for Private Educational Facilities (Registration and Authorization)

1. Application and Proof of Enrollment, Authorization, Incorporation of new subjects or courses and/or Renewal of Authorization to Operate the Institution.
2. Copy of Identity Card of the applicant, if naturalized citizen (copy of the Official Gazette of the Naturalization Act).
3. Copy of the Single Tax Information Registry (RIF) of the Institution.
4. Copy of the proof of registration with the Ministry of People’s Power for Education (Basic, Diversified High School and Professional Education).
5. Copy of proof of approval of the Eponymous certificate issued by the Ministry of People’s Power for Education.
6. Certified copy of the Constitutive Document of the Institution.
7. Copy of the last duly registered Assembly of Partners or Associates in the case of a legal entity.
8. Structural organization chart of the Institution.
9. Copy of the Registry of Industry, Patent and Commerce of the property for educational use.
10. Copy of proof of registration and solvency of employer or company, issued by the Venezuelan Institute of Social Security (IVSS).
11. Copy of the Declaratory Personal Accident Policy or Medical Care Service.
12. Copy of proof of compliance with fire prevention regulations issued by the Fire Department.
13. Copy of the sanitary permit issued by the Ministry of People’s Power for Health (MPPS).
14. Copy of Income Tax Solvency (ISLR).
15. The programs, study plans and schedules of the nautical subjects or courses to impart with its respective manual of the instructor and the participant.
16. Inventory and description of office furniture and equipment.
17. Inventory and description of resources, printed and non-printed didactic materials for learning in nautical education.
18. List of management personnel, teachers and instructors, with their current curriculum vitae and the corresponding annexed supports.
19. Copy of the authorization certificate for the use of the facilities necessary for the practices corresponding to the programs that require it.
20. Internal Regulations of the Campus (Norms of coexistence) that include the information of the school suit, in accordance with the provisions of article 6 of the joint resolution of the Ministry of the Popular Power for Education and the Ministry of the Popular Power for Infrastructure, published in Official Gazette No. 38.538 dated October 6, 2006.
21. Educational project to administer the Middle Technician Curriculum, Options: Navigation and Machinery.
22. Quality Management System implemented (Institution’s Standards and Procedures Manual).
23. Onboard internship plan.
24. Payment to the bank account of Banco Banesco, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEA PEOPLE PROCEDURES PAYMENT TABLE (located next to the TYPE OF APPLICATION). This table will be updated at the DICOM rate every Monday at noon. *The date of the payment made must correspond with the date of the application and is valid for three months after it is made. According to the following procedures: 417 € for the Initial Supervision of the Registration and Operating Permit (Article 62, Item 1); and 833 € for the Issuance of the Operating Permit (Article 62, Item 4).
25. The payment settlement form to INEA will be uploaded to each user in his SEAFOOD file, in the OBSERVATIONS tab, by the Seafarer’s Management staff once the payment has been made.
