CERTIFICATE OF PROFICIENCY FOR ENGINE CADET (RENEWAL)
| 1. | Identity Card. |
| 2. | Passport (optional). |
| 3. | High-resolution and clear photograph, red background, Uniform No. 2 (epaulettes with 5 stripes of 7 millimeters, both shoulders and face visible, updated no more than six months ago, neutral facial expression, no patterns in the beard for men, no low-cut necklines for women, and no photo montages). It must be attached in the SEAFARER FILE section. |
| 4. | Expired Document (Certificate of Proficiency for Engine Cadet, both sides). |
| 5. | Original Cadet Certificate issued by the educational institution. |
| 6. | IMO Courses (required and additional, both sides, including registration and folio numbers). The validity of the requested document depends on the date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab. |
| 7. | Form P. |
| 8. | Maritime Medical Certificate (physical and mental fitness with vision and hearing specifications), in the Maritime Medicine format available on the INEA website (must be performed by an INEA-authorized physician or an Internist from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician). |
