RENEWAL OF RECREATIONAL FREE-DIVING (APNEA) DIVER CREDENTIAL
| 1. | Identity Card |
| 2. | High-resolution, sharp photograph, blue background, formal attire (both shoulders and face visible, updated within the last six months, neutral facial expression, no glasses, no groomed patterns in the beard for men, no low-cut necklines for women, no photomontages). Must be attached in the SEAFARER’S FILE section. |
| 3. | Expired document (Recreational free-diving (apnea) diver credential, both sides). Must be attached in the SEAFARER’S FILE section, under the CERTIFICATES tab. |
| 4. | Certificate of refresher competency (in case of not having practiced diving activities for 6 months). |
| 5. | Medical report of the Diver (must be performed by a Physician authorized by INEA or an Internist from the Public or Private Network). |
| 6. | Payment for document issuance, to the Banesco Bank Current Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE as per Article 61, Item 20 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and is valid for three months once it is performed”. |
| 7. | INEA payment settlement form will be uploaded to each user in their “SEAFARER’S FILE”, under the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
Note: Applications before INEA shall be carried out without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
