COURSE INSTRUCTOR CERTIFICATE (RENEWAL OR INCORPORATION OF COMPETENCIES)
- Identity Card.
- Updated Curriculum Vitae (CV)
- High-resolution and sharp photograph, blue or red background depending on the specialty, uniform No. 2 (stripes/epaulets depending on the specialty), blue background, formal attire (both shoulders and face visible, updated within the last six months, neutral facial expression, no patterns or designs in the beard for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER SERVICE RECORD section.
- Expired document (Course Instructor Certificate subject to updating or renewal).
- Credential (both sides) or Merchant Marine Certificate. It must be attached in the SEAFARER SERVICE RECORD section, under the CERTIFICATES tab (optional).
- University Degree, other studies, and courses that are not IMO.
- List of courses in the aquatic field that you wish to teach.
- The IMO Courses (required and additional, both sides, with registration and folio); the validity of the requested document depends on the date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab.
- Seafarer’s Discharge Book, all Merchant Mariners. (all pages utilized).
- Maritime Medical Certificate (physical and mental fitness specifying vision and hearing), in the Maritime Medicine format found on the INEA website, including Audiometry and Phoniatry studies (must be performed by an INEA-authorized Physician or an Internist from the Public Health Network).
- Payment for document issuance, to the Checking Account of Banco Banesco, under the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS PROCEDURES PAYMENT TABLE; if it is a Renewal, according to Article 59, Item 4; if it is an Updating of each new competency (for each one), according to Article 59, Item 13 (located next to the TYPE OF REQUEST). This Table will be updated to the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the request and has three months of validity once performed.”
- The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE”, under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
