TEMPORARY PERMIT FOR FIRST DECK OFFICER (ORIGINAL)

1. Identity Card.
2. Passport (Optional).
3. High-resolution, sharp photograph, blue background, Uniform No. 2 (epaulets with 3 fourteen-millimeter stripes, both shoulders and face visible, updated within the last six months, neutral facial expression, no patterns in the beard for men, and no low-cut necklines for women, no photomontages). It must be attached in the SEAFARER FILE section.
4. Credential (both sides) or Second Deck Officer License. It must be attached in the SEAFARER FILE section, under the LICENSES tab.
5. Shipowner’s Letter Requesting the Temporary Permit.
6. The IMO Courses (required and additional, both sides, including registration and folio numbers). The validity of the requested document depends on the date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab.
7. Certificate of Sea Service (24 months of sea time served under the Second Deck Officer License).
8. Seafarer Book (all used pages).
9. Maritime Medical Certificate (physical and mental fitness with vision and hearing specification), in the Maritime Medicine format found on the INEA website (must be performed by a Physician authorized by the INEA or an Internist Physician from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).
10. Payment for document issuance fee, to the Banesco Bank Current Account, under the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE pursuant to Article 59, Item 7 (located next to APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and remains valid for three months once it has been processed”.
11. The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE,” under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
