CERTIFICATE OF PROFICIENCY FOR SUPPORT LEVEL PERSONNEL (COPY)
| 1. | Identity Card. |
| 2. | High-resolution, sharp photograph with a blue background, in formal attire (shoulders and face visible, taken within the last six months, neutral facial expression, no groomed patterns in the beard for men, and no low-cut necklines for women; no photomontages allowed). It must be attached in the SEAFARER FILE section. |
| 3. | In the case of a copy due to loss, theft, or robbery, if the user does not have a record of the document in SINEA, a PDF copy of the Certificate of Proficiency for Support Level Personnel with a manual signature must be scanned if available; otherwise, a report filed with the competent judicial authority must be uploaded. In the case of a copy due to damage or deterioration, scan and submit the affected Certificate or an explanatory letter of the events occurred. |
| 4. | Certificate of Proficiency for Support Level Personnel. It must be attached in the SEAFARER FILE section, under the CERTIFICADOS tab. |
| 5. | Payment for document issuance, to the Banesco Bank checking account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE as per Article 59, Item 12 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the request and is valid for three months once it is carried out” . |
| 6. | INEA payment settlement form will be uploaded to each user in their “SEAFARER’S FILE”, under the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
