    CERTIFICATE OF PROFICIENCY FOR SUPPORT LEVEL PERSONNEL
    (ORIGINAL)

    1. Identity Card.
    2. Passport (Optional).
    3. High-resolution and sharp photograph, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, neutral facial expression, no patterns in the beard for men, and no low-cut necklines for women, no photo montages). It must be attached in the SEAFARER FILE section.
    4. IMO Courses (required and additional, both sides, with registration and folio). The validity of the requested document depends on the date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab.
    5. Seafarer’s Identity Document (all used pages) or Q1 Form.
    6. Maritime Medical Certificate (physical and mental fitness specifying vision and hearing), in the Maritime Medicine format found on the INEA website (must be performed by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).
    7. Payment for document issuance, to the Banesco Bank checking account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE as per Article 59, Item 5 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the request and is valid for three months once it is carried out”.
    8. INEA payment liquidation forms will be uploaded for each user in their “SEAFARER’S FILE,” under the “REMARKS” tab by the Seafarer Management personnel once the payment has been confirmed.

    Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
