COURSE INSTRUCTOR CERTIFICATE (COPY)
- Identity Card.
- High-resolution and sharp photograph, blue or red background depending on the specialty, uniform No. 2 (stripes/epaulets depending on the specialty), blue background, formal attire (both shoulders and face visible, updated within the last six months, neutral facial expression, no patterns or designs in the beard for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER SERVICE RECORD section.
- In the case of a replacement copy due to misplacement, theft, or robbery, if the user does not have a record of the document in the SINEA, a copy of the Course Instructor Certificate must be scanned in PDF format, including a handwritten signature if available; otherwise, a formal complaint filed before the competent judicial authority must be uploaded. In the case of a replacement copy due to damage or deterioration, the affected Certificate or an explanatory letter detailing the events that occurred must be scanned and submitted.
- Credential (both sides) or Merchant Marine Certificate. It must be attached in the SEAFARER SERVICE RECORD section, under the CERTIFICATES tab (optional).
- Payment for document issuance, to the Banesco current account, held by INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER TRANSACTION FEES TABLE pursuant to Article 59, Item 12 (located next to REQUEST TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and is valid for three months once processed.”
- The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE”, under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
