MOBILE OFFSHORE DRILLING UNIT CERTIFICATE (MODU CODE) (COPY)
- Identity Card.
- Photograph with good resolution and clarity, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, neutral facial expression, without glasses, no beard styling or patterns for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section.
- In the case of a replacement copy due to misplacement, theft, or robbery, if the user does not have a record of the document in the SINEA, a copy of the Mobile Offshore Drilling Unit Certificate (MODU) must be scanned in PDF format, including a handwritten signature if available; otherwise, a formal complaint filed before the competent judicial authority must be uploaded. In the case of a replacement copy due to damage or deterioration, the affected Certificate or an explanatory letter detailing the events that occurred must be scanned and submitted.
- Payment for the concept of document issuance, to the Current Account (Cuenta Corriente) of Banco Banesco, under the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts specified in the SEAFARER PROCEDURES PAYMENT TABLE pursuant to Article 59, Item 12 (located next to APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the application and remains valid for three months once processed.”
- The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE”, under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
