RECREATIONAL DIVING INSTRUCTOR CREDENTIAL / BASIC SPORT (COPY)
| 1. | Identity Card. |
| 2. | High-resolution, sharp photograph, blue background, formal attire (both shoulders and face visible, updated within the last six months, neutral facial expression, no glasses, no groomed patterns in the beard for men, no low-cut necklines for women, no photomontages). Must be attached in the SEAFARER’S FILE section. |
| 3. | In the case of a copy due to loss, theft, or robbery, if the user does not have a record of the document in SINEA, they must scan a PDF copy of the Recreational / Basic Sport Diving Instructor Credential, including a manual signature if available; otherwise, they must upload a report filed with the competent judicial authority. In the case of a copy due to damage or deterioration, the affected Credential or an explanatory letter of the events occurred must be scanned and submitted. |
| 4. | Recreational Diving Instructor Credential / Basic Sport (both sides). It must be attached in the SEAFARER’S FILE section, under the CERTIFICATES tab. |
| 5. | Payment for document issuance, to the Banesco Bank Current Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE as per Article 61, Item 21 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and is valid for three months once it is performed”. |
| 6. | INEA payment settlement form will be uploaded to each user in their “SEAFARER’S FILE”, under the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
