ORIGINAL RECREATIONAL / ADVANCED SPORTS DIVING INSTRUCTOR CREDENTIAL
| 1. | Identity Card. |
| 2. | Updated Curriculum Vitae (Spanish). |
| 3. | High-resolution and sharp photograph, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, with no facial expressions, no glasses, no designs in the beard for men, and no low-cut necklines for women, nor photomontages). It must be attached in the SEAFARER’S FILE section. |
| 4. | International Certification or Recreational / Basic Sports Diving Instructor Credential (both sides). |
| 5. | First Aid Instructor Course or IMO Courses: 1.13 and 1.19 (valid, both sides, with registration and folio). They must be attached in the SEAFARER’S FILE section, under the COURSES tab. |
| 6. | Teaching Component / Teacher Training / Teacher Education (minimum 200 hours). |
| 7. | Certificate of affiliation with the Certifying Agency (PADI, CMAS, WRSTC, RSTC, among others). |
| 8. | Maritime Medical Certificate (physical and mental fitness with vision and hearing specifications), in the Maritime Medicine format available on the INEA website (must be performed by an INEA-authorized physician or an Internist from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician). . |
| 9. | Payment for the issuance of the document, to the Banesco Bank checking account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE as per Article 61, Item 18 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and has three months of validity once it is performed”. |
| 10. | The INEA payment liquidation form will be uploaded to each user’s “SEAFARER’S FILE,” in the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
