SURFACE-SUPPLIED AIR DIVER’S TENDER CREDENTIAL (COPY)
- Identity Card.
- Photograph with good resolution and clarity, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, neutral facial expression, without glasses, no beard styling or patterns for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section.
- In the case of a copy due to misplacement, theft, or robbery, if the user does not have the document registered in the SINEA system, they must scan a PDF copy of the Surface-Supplied Air Diver’s Tender Credential, including a handwritten signature if available; otherwise, they must upload the official report filed before the competent judicial authority. In the case of a copy due to damage or deterioration, they must scan and submit the affected Credential or an explanatory letter of the events that occurred.
- Surface-Supplied Air Diver’s Tender Credential (both sides). It must be attached in the SEAFARER FILE section, under the CERTIFICATES tab.
- Payment for document issuance, to the Banesco Bank Checking Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE pursuant to Article 61, Item 21 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the request and is valid for three months once completed”.
- The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE”, under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
