ENDORSEMENT OF FISHING OFFICER LICENSE (COPY)
| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, with a neutral facial expression, without patterns in the beard for men, and without low-cut necklines for women, nor photo montages). It must be attached in the SEAFARER FILE section. |
| 3. | In the case of a copy due to misplacement, theft, or robbery, if the user does not have a record of the document in SINEA, a PDF copy of the Endorsement of Fishing Officer License with a manual signature must be scanned, if available; otherwise, a report filed with the competent judicial authority must be uploaded. In the case of a copy due to damage or deterioration, the affected Endorsement or an explanatory letter of the events that occurred must be scanned and submitted. . |
| 4. | Credential (both sides) or Fishing Officer Certificate. This must be attached in the SEAFARER FILE section, under the CERTIFICATES tab. |
| 5. | Payment for document issuance, to the Banesco Current Account in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE as per Article 59, Item 12 (located next to the APPLICATION TYPE). This table is updated to the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and is valid for three months once performed.” |
| 6. | INEA payment settlement form will be uploaded to each user in their “SEAFARER’S FILE”, under the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
