| 1. | Identity Card. |
| 2. | High-resolution, sharp photograph, blue background (deck/navigation) or red background (engine), formal suit (both shoulders and face visible, updated within the last six months, neutral facial expression, no patterns in the beard for men, and no low-cut necklines for women, no photomontages). It must be attached in the SEAFARER FILE section. |
| 3. | Certificate of completion of military service. |
| 4. | IMO Courses (required, both sides, with registry and folio numbers). The validity of the requested document depends on the expiration date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab. |
| 5. | Maritime Medical Certificate (physical and mental fitness with specific evaluation of vision and hearing), in the Maritime Medicine format available on the INEA website (must be issued by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be issued by a Cardiologist, Internist, Intensivist, or Emergency Physician). |
| 6. | Payment for document issuance fee, to the Banesco Bank Current Account, under the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE pursuant to Article 59, Item 9 (located next to APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and remains valid for three months once it has been processed”. |
| 7. | The INEA payment liquidation form will be uploaded to each user’s SEAFARER FILE, under the OBSERVATIONS tab, by the staff of the Seafarer Management Department once their payment has been confirmed. . |

Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
