ENDORSEMENT OF THE THIRD ENGINEER OFFICER CERTIFICATE (COPY)
| 1. | Identity Card. |
| 2. | High-resolution, sharp photograph, red background, Uniform No. 2 (epaulettes with one 14-millimeter stripe, both shoulders and face visible, updated within the last six months, neutral facial expression, no styled or shaped beard patterns for men, no low-cut necklines for women, and no photomontages). This must be attached in the SEAFARER FILE section. |
| 3. | In the case of a copy due to loss, theft, or robbery, if the user does not have a record of the document in the SINEA, a PDF scan of the Endorsement of the Third Engineer Officer Certificate with a manual signature must be uploaded if available; otherwise, a formal report filed with the competent judicial authority must be uploaded. In the case of a copy due to damage or deterioration, scan and submit the affected Endorsement or an explanatory letter of the events occurred. |
| 4. | Credential (both sides) or Third Engineer Officer Certificate. This must be attached in the SEAFARER FILE section, under the CERTIFICATES tab. |
| 5. | Payment for document issuance, to the Banesco Current Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to Article 59, Item 12 (located next to the APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and remains valid for three months once processed”. |
| 6. | INEA payment settlement form will be uploaded to each user in their “SEAFARER’S FILE”, under the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
