This must be attached in the SEAFARER FILE section. |

IMO Courses (required and additional, both sides, with registration and folio numbers). The validity of the requested document depends on the completion dates of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab. |

Maritime Medical Certificate (physical and mental fitness with vision and hearing specifications), in the Maritime Medicine format found on the INEA website (must be issued by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician). |

Payment for the document issuance fee, to the Banesco Current Account in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURE PAYMENT TABLE according to Article 59, Item 1 (located next to the TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and is valid for three months once completed.” |

The INEA payment liquidation form will be uploaded to each user’s “SEAFARER’S FILE,” under the “OBSERVATIONS” tab by the Seafarer Management personnel once the payment has been confirmed. |
