COPY OF THIRD NAVIGATION OFFICER TITLE CREDENTIAL

| 2. | Updated photograph with good resolution and clarity, blue background, Uniform No. 2 without cap (epaulettes with one 14 mm stripe), or formal attire (both shoulders and face visible, no glasses, no facial expressions, no shaped beards for men, no low-cut necklines for women, and no photo montages). Must be attached in the SEAFARER FILE section. |
| 3. | In the event of damage, deterioration, loss, theft, or robbery, if the user does not have a record of the document in the SINEA, a PDF copy of the Third Navigation Officer Title Credential with a manual signature must be scanned, if available. Otherwise, a report filed with the competent judicial authority or an explanatory letter detailing the events and providing the information of the requested document must be uploaded. |
| 4. | Payment for document issuance, to the Banco Banesco checking account, under the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE as per Article 61, Item 21 (located next to the APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The payment date must correspond to the application date and is valid for three months once made”. |
| 5. | The INEA payment liquidation form will be uploaded to each user’s SEAFARER’S FILE, under the OBSERVATIONS tab, by the Seafarer Management personnel once the payment has been confirmed. |
