THIRD NAVIGATION OFFICER TITLE AND CREDENTIAL (ORIGINAL)
| 1. | Identity Card. |
| 2. | Updated photograph with good resolution and clarity, blue background, Uniform No. 2 without cap (epaulettes with one 14 mm stripe), or formal attire (both shoulders and face visible, no glasses, no facial expressions, no shaped beards for men, no low-cut necklines for women, and no photo montages). Must be attached in the SEAFARER FILE section. |
| 3. | Marine Engineering Academic Transcript (including completion of internships) and Graduation Certificate. |
| 4. | Sea Service Certificate (12 months of sea time served as an intern). |
| 5. | Form P (both sides). |
| 6. | Maritime Medical Certificate (physical and mental fitness specifying vision and hearing), in the Maritime Medicine format available on the INEA website (must be issued by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and electrocardiogram (must be issued by a Cardiologist, Internist, Intensivist, or Emergency Physician). |
| 7. | Payment for document issuance, to the Banco Banesco checking account, under the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE as per the following Articles: Title: Article 59, Item 1 and Credential: Article 61, Item 4 (located next to the APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The payment date must correspond to the application date and is valid for three months once made”. |
| 8. | The INEA payment settlement form will be uploaded for each user to their SEAFARER FILE, under the OBSERVATIONS tab, by the Seafarers Management personnel once the payment has been confirmed. |
