COPY OF YACHT CAPTAIN LICENSE

1.	Identity Card.
2.	Updated photograph with high resolution and sharpness, blue background, formal attire (both shoulders and face visible, no glasses, no facial expressions, no shapes in the beard for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER’S FILE section.
3.	In the event of damage, deterioration, loss, theft, or robbery, if the user does not have a record of the document in the SINEA, a PDF copy of the Yacht Captain License with a manual signature must be scanned, if available. Otherwise, a report filed before the competent judicial authority or an explanatory letter of the events must be uploaded, indicating the details of the requested document.
4.	Payment for document issuance, to the Banco de Venezuela checking account, in the name of INEA, 0102-0762-25-0000003188, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to Article 61, Item 21 (located next to TYPE OF REQUEST). This Table is updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and has three months of validity once it is performed”.
5.	The INEA payment liquidation form will be uploaded to each user’s SEAFARER’S FILE, under the OBSERVATIONS tab, by the Seafarer Management personnel once the payment has been confirmed.

Note: All documents must be legible, digitized in PDF format, and attached in the SINEA.
