Maritime Medical Certificate (physical and mental fitness with vision and hearing specifications), in the Maritime Medicine format found on the INEA website (must be issued by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician). |

Payment for document issuance, made to the Banesco Checking Account, under the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE pursuant to Article 59, Item 2 (located next to the TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the request and is valid for three months once processed”. |

The INEA payment liquidation form will be uploaded to each user’s “SEAFARER’S FILE,” under the “OBSERVATIONS” tab by the Seafarer Management personnel once the payment has been confirmed. |
