CERTIFICATE OF ENDORSEMENT FOR SECOND-CLASS MOTORMAN (RENEWAL)
| 1. | Identity Card. |
| 2. | Passport (Optional). |
| 3. | High-resolution, sharp photograph, red background, Uniform No. 2 (shoulder boards with 1 five-millimeter stripe, both shoulders and face visible, updated within the last six months, with a neutral facial expression, no groomed patterns/shapes in the beard for men, and no plunging necklines for women, no photomontages). This must be attached in the SEAFARER FILE section. |
| 4. | Expired document (Certificate of Endorsement for Second-Class Motorman, both sides). |
| 5. | Credential (both sides) or Second-Class Motorman Certificate. This must be attached in the SEAFARER FILE section, under the CERTIFICATES tab. . |
| 6. | IMO Courses (required and additional, both sides, including registration and folio numbers). The validity of the requested document depends on the date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab. |
| 7. | Seafarer’s Identity Document (all used pages). |
| 8. | Maritime Medical Certificate (physical and mental fitness with vision and hearing specifications), in the Maritime Medicine format available on the INEA website (must be performed by an INEA-authorized physician or an Internist from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician). |
| 9. | Payment for document issuance, to the Banesco Current Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to Article 59, Item 12 (located next to the APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and remains valid for three months once processed”. . |
| 10. | The INEA payment liquidation form will be uploaded to each user’s “SEAFARER’S FILE,” in the “OBSERVATIONS” tab by the Seafarers’ Management personnel once their payment has been confirmed. |
