    ORIGINAL ENDORSEMENT OF MASTER MARINER TITLE

    GRAN MISIÓN TRANSPORTE VENEZUELA LETTERHEAD

    ENDORSEMENT OF MASTER MARINER TITLE (ORIGINAL)

    1. Identity Card.
    2. Passport (Optional).
    3. Photograph with good resolution and clarity, blue background, Uniform No. 2 (epaulettes with 4 stripes of 14 millimeters, both shoulders and face visible, updated within the last six months, no facial expressions, no designs in the beard for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE (EXPEDIENTE DE MARINO) section.
    4. Credential (both sides) or Master Mariner Title. It must be attached in the SEAFARER FILE (EXPEDIENTE DE MARINO) section, under the TITLES (TÍTULOS) tab.
    5. IMO Courses (required and additional, both sides, with registration and folio numbers). The validity of the requested document depends on the completion dates of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab.
    6. Seafarer’s Identity Document (all used pages).
    7. Maritime Medical Certificate (physical and mental fitness with vision and hearing specifications), in the Maritime Medicine format found on the INEA website (must be issued by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).
    8. Payment for document issuance to the Banesco Bank current account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS’ PROCEDURE PAYMENT TABLE (TABLA DE PAGO DE TRÁMITES DE GENTE DE MAR) according to Article 59, Item 1 (located next to the APPLICATION TYPE). This table will be updated to the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application and is valid for three months once performed”.
    9. The INEA payment liquidation form will be uploaded to each user’s “SEAFARER’S FILE,” under the “OBSERVATIONS” tab by the Seafarer Management personnel once the payment has been confirmed.
