ORIGINAL FISHING CHIEF ENGINEER LICENSE AND CREDENTIAL
| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, red background, formal attire (both shoulders and face visible, no more than six months old, neutral facial expression, no designs in facial hair for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section. |
| 3. | Fishing Marine Engineer Officer License Credential (both sides) or Fishing Marine Engineer Officer License. It must be attached in the SEAFARER FILE section, under the TITLES tab. |
| 4. | Degree of Higher University Technician in Naval Technology, Major: Naval Mechanics. |
| 5. | Fishing Chief Engineer Course Completion Certificate. |
| 6. | Sea Time Certificate (12 months of sea service with the Fishing Marine Engineer Officer License). |
| 7. | Seafarer’s Identity Document (all pages used). |
| 8. | Maritime Medical Certificate (physical and mental fitness with vision and hearing specifications), in the Maritime Medicine format available on the INEA website (must be performed by an INEA-authorized physician or an Internist from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician). . |
| 9. | Document issuance payment to the Banesco checking account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURE PAYMENT TABLE according to the following Articles: License: 59, Item 2 and Credential: 61, Item 9 (located next to TYPE OF REQUEST). This table is updated to the DICOM rate every Monday at noon. “The date of the payment must correspond to the date of the application and remains valid for three months once processed”. |
| 10. | The INEA payment liquidation form will be uploaded to each user’s SEAFARER FILE, under the REMARKS tab, by the Seafarers’ Management personnel once the payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
