RECREATIONAL DIVING INSTRUCTOR CREDENTIAL / ADVANCED SPORT (COPY)
| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, red background, formal attire (both shoulders and face visible, updated no more than six months ago, no facial expressions, no shapes in the beard for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER’S FILE section. |
| 3. | In the case of a copy due to loss, theft, or robbery, if the user does not have a record of the document in SINEA, a PDF copy of the Fishing Engine Officer Official Credential must be scanned, including a manual signature if available; otherwise, a report filed with the competent judicial authority must be uploaded. In the case of a copy due to damage or deterioration, the affected Credential or an explanatory letter of the events occurred must be scanned and submitted. |
| 4. | Associate Degree in Naval Technology, major: Naval Mechanics. |
| 5. | Payment for document issuance, to the Banesco Bank checking account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to Article 61, Item 21 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and has three months of validity once it is performed”. |
| 6. | The INEA payment liquidation form will be uploaded to each user’s SEAFARER’S FILE, under the OBSERVATIONS tab, by the Seafarer Management personnel once the payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
