FISHING MASTER TITLE AND CREDENTIAL (ORIGINAL)
| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, with a neutral facial expression, without patterns in the beard for men, and without low-cut necklines for women, nor photo montages). It must be attached in the SEAFARER FILE section. |
| 3. | Fishing Officer Title Credential (both sides) or Fishing Officer Title. It must be attached in the SEAFARER FILE section, under the TITLES tab. |
| 4. | Associate Degree in Naval Technology, major: Navigation and Fishing. |
| 5. | Original Fishing Master Course Completion Certificate. |
| 6. | Sea Service Certificate (12 months served under a Fishing Officer Title). |
| 7. | Seafarer’s Identity Document (all used pages). |
| 8. | Maritime Medical Certificate (physical and mental fitness with vision and hearing specifications), in the Maritime Medicine format available on the INEA website (must be performed by an INEA-authorized physician or an Internist from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician). . |
| 9. | Payment for document issuance, to the Banesco Bank checking account, under the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE as per the following Articles: Title: 59, Item 2 and Credential: 61, Item 5 (located next to the APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The payment date must correspond to the application date and is valid for three months once made”. |
| 10. | The INEA payment settlement form will be uploaded for each user to their SEAFARER FILE, under the OBSERVATIONS tab, by the Seafarers Management personnel once the payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
