ISSUANCE OF SEAFARER’S IDENTITY DOCUMENT FOR SUPPORT PERSONNEL (ORIGINAL)
| 1. | Identity Card. |
| 2. | Updated photograph with high resolution and sharpness, blue background, formal attire (both shoulders and face visible, no glasses, no facial expressions, no shapes in the beard for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER’S FILE section. |
| 3. | Form Q-1, or Form Q-2 (reserve personnel with no onboard experience), both with a minimum of 9 months of sea service; or a certificate of 12,000 nautical miles sailed if the applicant is a Navy reservist with onboard experience, highlighting their specialty. |
| 4. | Payment for the document issuance fee, made to the Current Account of Banco de Venezuela, under the name of INEA, 0102-0762-25-0000003188, RIF G-20002141-1, in accordance with the amounts specified in the SEAFARER TRANSACTION FEES TABLE pursuant to Article 60, Item 4 (located next to APPLICATION TYPE). This table is updated according to the DICOM rate every Monday at noon. “The date of the payment made must match the application date and remains valid for three months once processed”. . |
| 5. | The INEA payment liquidation form will be uploaded to each user’s SEAFARER’S FILE, under the OBSERVATIONS tab, by the Seafarer Management personnel once the payment has been confirmed. |
Note: All documents must be valid, legible, digitized in PDF format, and attached to the SINEA.
Note: To collect the Seafarer Book at the Port Captaincies, users must print Form J and present it at the Port Captaincy where they submitted their application, attaching 2 passport-sized photographs with the same characteristics as those described in the requirements.
