| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, blue background (deck/navigation) or red background (engine) according to specialty, with Uniform No. 2 and visible stripes if the applicant is a cadet, or formal attire if a trainee (with both shoulders and face visible, updated within the last six months, with a neutral facial expression, no styled or patterned beard for men, and no low-cut necklines for women, nor photomontages). This must be attached in the SEAFARER FILE section. |
| 3. | Letter of request for the issuance of Form “P” submitted by the University or Nautical Institute, addressed to the Harbor Master. |
| 4. | IMO Courses (required, both sides, with registry and folio numbers). The validity of the requested document depends on the expiration date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab. |
| 5. | Maritime Medical Certificate (physical and mental fitness, specifically detailing vision and hearing), in the Maritime Medicine format available on the INEA website (must be issued by a physician authorized by INEA or an Internist from the Public Health Network). Applicants over 40 years of age must submit a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician). |
