ENDORSEMENT OF RECOGNITION OF TITLE OF COASTAL CAPTAIN (ORIGINAL)
- Identity Card.
- Passport (Optional).
- High-resolution and sharp photograph, blue background, in uniform (rank must be visible on both shoulders, updated within the last six months, neutral facial expression, no glasses, no groomed patterns/shapes in the beard for men, and no low-cut necklines for women, no photomontages). This must be attached in the SEAFARER FILE section.
- Credential (both sides) or Coastal Captain Certificate issued by the Maritime Administration of the applicant’s nationality and duly verifiable through available means. This must be attached in the SEAFARER FILE section, under the CERTIFICATES tab.
- IMO Courses duly approved by a Nautical Educational Institution registered with the Venezuelan Maritime Administration (required and additional, both sides, including registry and folio number). The validity of the requested document depends on the issuance date of said courses. These must be attached in the SEAFARER FILE section, under the COURSES tab.
- Seafarer Book (all used pages).
Maritime Medical Certificate (physical and mental fitness with vision and hearing specification), in the Maritime Medicine format found on the INEA website (must be performed by a Physician authorized by the INEA or an Internist Physician from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).- Payment for the issuance of the document, to the bank account of Banco Banesco, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the SEA PEOPLE PROCEDURES PAYMENT TABLE according to Article 59, Item 1 (located next to the TYPE OF APPLICATION). This Table will be updated at the DICOM rate every Monday at noon. *The date of the payment made must correspond with the date of the application and is valid for three months after it is made “*.
- The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE,” under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
