| 1. | Identity Card. |
| 2. | High-resolution, sharp photograph, blue background (deck/navigation) or red background (engine), formal suit (both shoulders and face visible, updated within the last six months, neutral facial expression, no patterns in the beard for men, and no low-cut necklines for women, no photomontages). It must be attached in the SEAFARER FILE section. |
| 3. | OMI Courses (required, both sides, with registration and folio). The validity of the requested document depends on the date of the courses. They must be attached in the section MARINE’S EXPEDIENT, in the tab COURSES. |
| 4. | Maritime Medical Certificate (physical and mental fitness specifying vision and hearing), in the Maritime Medicine format available on the INEA website (must be issued by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and electrocardiogram (must be issued by a Cardiologist, Internist, Intensivist, or Emergency Physician). |
| 5. | Payment for the issuance of the document, to the bank account of Banco Banesco, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, according to the amounts indicated in the TABLE OF PAYMENT OF SEA PEOPLE PROCEDURES according to Article 59, Item 9 (located next to TYPE OF APPLICATION). This Table will be updated at the DICOM rate every Monday at noon. *The date of the payment made must correspond with the date of the application and is valid for three months after it is made “*. |
| 6. | The INEA payment liquidation form will be uploaded to each user’s SEAFARER’S FILE, under the OBSERVATIONS tab, by the Seafarer Management personnel once the payment has been confirmed. |

Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
