FORM P (RENEWAL)
| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, blue background (deck/navigation) or red background (engine) according to specialty, with Uniform No. 2 and visible stripes if the applicant is a cadet, or formal attire if a trainee (with both shoulders and face visible, updated within the last six months, with a neutral facial expression, no styled or patterned beard for men, and no low-cut necklines for women, nor photomontages). This must be attached in the SEAFARER FILE section. |
| 3. | Expired document (Form P). |
| 4. | Letter of request of the emission of the Form “P” presented by the University or Nautical Institute directed to the Captain of Port. |
| 5. | OMI Courses (required, both sides, with registration and folio). The validity of the requested document depends on the date of the courses. They must be attached in the section MARINE’S EXPEDIENT, in the tab COURSES. |
| 6. | Maritime Medical Certificate (physical and mental fitness specifying vision and hearing), in the Maritime Medicine format available on the INEA website (must be issued by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and electrocardiogram (must be issued by a Cardiologist, Internist, Intensivist, or Emergency Physician). |

Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
