RENEWAL OF SURFACE-SUPPLIED AIR DIVING SUPERVISOR CREDENTIAL
SURFACE-SUPPLIED AIR DIVING SUPERVISOR CREDENTIAL (RENEWAL)
- Identity Card.
- Updated Curriculum Vitae (Spanish) with Diving Logbook (Hours and/or Activities), demonstrating in the Logbook 100 days of Deck operation specifying the use of surface-supplied equipment within the last year, 150 cumulative diving hours within the last year, and 30 days as an endorsed Supervisor (after the last certification issued by INEA).
- Photograph with good resolution and clarity, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, neutral facial expression, without glasses, no beard styling or patterns for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section.
- Expired document (Surface-Supplied Air Diving Supervisor Credential, both sides). It must be attached in the SEAFARER FILE section, under the CERTIFICATES tab.
- First Aid Instructor Course (current/valid). These must be attached in the SEAFARER FILE (EXPEDIENTE DE MARINO) section, under the COURSES (CURSOS) tab.
- Module C Course (Supervisory level) (current/valid). These must be attached in the SEAFARER FILE (EXPEDIENTE DE MARINO) section, under the COURSES (CURSOS) tab.
- IMO Courses (required, both sides, with registration and folio numbers). The validity of the requested document depends on the date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab.
- Employment reference(s) indicating the specific diving position held.
Maritime Medical Certificate (physical and mental fitness with vision and hearing specification), in the Maritime Medicine format found on the INEA website (must be performed by a Physician authorized by the INEA or an Internist Physician from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).- Payment for document issuance, to the current account at Banesco, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS PROCEDURE PAYMENT TABLE pursuant to Article 61, Item 19 (located next to the APPLICATION TYPE). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the application, and it is valid for three months once processed.”
- The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE”, under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.

Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
