Requirements for a copy of an authorized Medical Doctor Certificate

1. Identity card (legible and valid).
2. Recent photograph with good resolution and clarity, blue background, formal attire (both shoulders and face visible, without glasses, neutral facial expression, no beard styling or patterns for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section.
3. In the event of damage, deterioration, misplacement, theft, or robbery, if the user does not have a record of the document in the SINEA system, a copy of the Maritime Medical Certificate with a manual signature must be scanned in PDF format if available; otherwise, a police report filed before the competent judicial authority or an explanatory letter of the events detailing the data of the requested document must be uploaded.
4. Submit all the aforementioned requirements to the Directorate of Seafarers via the following institutional email: MEDICINAMARITIMA@INEA.GOB.VE. Your documents will be evaluated there, and a response will be provided so you can initiate your application through this system (SINEA).
5. Payment for the procedure must be made once your documents have been evaluated and your application has been approved by the Directorate.
   Payment for document issuance, to the Checking Account (Cta. Cte.) of Banco de Venezuela, under the name of INEA, 0102-0762-25-0000003188, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS’ PROCEDURE PAYMENT TABLE (TABLA DE PAGO DE TRÁMITES DE GENTE DE MAR) according to the Articles: Certificate: Article 59, Item 4 and Credential: Article 61, Item 16 (located next to the TYPE OF REQUEST). The amounts in this payment table are expressed in EUROS and must be paid in national currency calculated at the BCV (Central Bank of Venezuela) exchange rate in effect on the day of payment. The full amount yielded by the BCV Value Date (Fecha Valor), including all 8 decimal places (e.g., XX.XXXXXXXX), must be used to calculate the payment. The payment has a validity of ninety (90) days, according to Circular No. 009 of 2018, published on the INEA website, www.inea.gob.ve.
