    REQUIREMENTS FOR THE ORIGINAL AUTHORIZED MEDICAL PRACTITIONER CERTIFICATE

    Requirements for Original Authorized Medical Practitioner Certificate

    1. Identity card (legible and valid).
    2. Updated Professional Resume (Spanish): Comprehensive curriculum vitae demonstrating a minimum of five (5) years of professional experience in the practice of medicine.
    3. Name, address, telephone number, and email address of the medical office.
    4. Recent photograph with good resolution and clarity, blue background, formal attire (both shoulders and face visible, without glasses, neutral facial expression, no beard styling or patterns for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section.
    5. Medical Degree on a black background (fondo negro).
    6. Having complied with Article 8 of the Law on the Practice of Medicine in the Bolivarian Republic of Venezuela.
    7. To be registered with the local Medical College, with a validity of three (3) months, and to provide a deontological certificate.

    9. Being familiar with the living and working conditions on board ships, through special instruction or possessing professional experience related to maritime activity (certificate or IMO courses). IMO courses must be attached in the MARINER’S RECORD section, under the COURSES tab.
    10. Professional Independence Letter: Professionally, the practitioner must be independent of shipowners, Seafarers, and their representatives when exercising their medical judgment regarding medical examination procedures. If employed by a maritime company, a Seafarer recruitment and placement agency, or working under contract for them, the working or employment conditions must guarantee that the evaluations carried out are based on regulatory, ethical, and deontological standards.
    11. Submit all the aforementioned requirements to the Directorate of Seafarers via the following institutional email: MEDICINAMARITIMA@INEA.GOB.VE. Your documents will be evaluated there, and a response will be provided so you can initiate your application through this system (SINEA).
    12. Payment for the procedure must be made once your documents have been evaluated and your application has been approved by the Directorate.
      Payment for document issuance, to the current account at Banco Venezuela, in the name of INEA, 0102-0762-25-0000003188, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS PROCEDURE PAYMENT TABLE pursuant to the following Articles: Certificate: 59, Item 4, and Credential: 61, Item 16 (located next to the APPLICATION TYPE). The amounts in this payment table are expressed in EUROS and must be paid in local currency, calculated at the BCV (Central Bank of Venezuela) exchange rate in effect on the day of payment. The full amount provided by the BCV Value Date, including all 8 decimal places (e.g., XX.XXXXXXXX), must be used to calculate the payment. The payment is valid for ninety (90) days, pursuant to Circular No. 009 of 2018, published on the INEA website, www.inea.gob.ve.
    Note: All documents must be valid, legible, digitized in PDF format, and attached to the SINEA.
