INDUSTRIAL DIVING INSTRUCTOR CREDENTIAL (COPY)

1. Identity Card.
2. Photograph with good resolution and clarity, blue background, formal attire (both shoulders and face visible, updated no more than six months ago, neutral facial expression, without glasses, no beard styling or patterns for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section.
3. In the case of a copy due to loss, theft, or robbery, if the user does not have a record of the document in the SINEA, a PDF copy of the Industrial Diving Instructor Credential with a manual signature must be scanned and submitted if available; otherwise, a formal complaint filed before the competent judicial authority must be uploaded. In the case of a copy due to damage or deterioration, the affected Credential or an explanatory letter of the events occurred must be scanned and submitted.
4. Industrial Diving Instructor Credential (both sides). It must be attached in the SEAFARER FILE section, under the CERTIFICATES tab.
5. Payment for document issuance, to the Banesco Bank Checking Account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARERS’ PROCEDURES PAYMENT TABLE pursuant to Article 61, Item 21 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond to the date of the request and is valid for three months once completed”.
6. The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE”, under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
