ELECTRO-TECHNICAL RATING CERTIFICATE OF PROFICIENCY (ORIGINAL)
REQUIREMENTS FOR ORIGINAL ISSUANCE
- Identity Card.
- Passport (Optional).
- High-resolution, sharp photograph with a red background, formal attire (both shoulders and face visible, updated within the last six months, neutral facial expression, no patterns or designs in beards for men, no low-cut necklines for women, and no photo montages). It must be attached in the SEAFARER FILE section.
- The IMO Courses (required and additional, both sides, including registration and folio numbers). The validity of the requested document depends on the date of these courses. They must be attached in the SEAFARER FILE section, under the COURSES tab.
- Seafarer’s Identification Document (all used pages) (with a minimum of 6 months of sea service in the engine area).
- Maritime Medical Certificate (physical and mental fitness with vision and hearing specification), in the Maritime Medicine format found on the INEA website (must be performed by a Physician authorized by the INEA or an Internist Physician from the Public Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).
- Payment for the document issuance fee, to the Banesco Bank current account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER TRANSACTION FEE TABLE according to Article 59, Item 5 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon.
. *The date of the payment made must correspond to the date of the application and has three months of validity once it is made*. - The INEA payment settlement form will be uploaded to each user’s “SEAFARER FILE,” under the “REMARKS” tab, by the Seafarer Management personnel once the payment has been confirmed.
