ORIGINAL FISHING ENGINE OFFICER TITLE AND CREDENTIAL
| 1. | Identity Card. |
| 2. | High-resolution and sharp photograph, red background, formal attire (both shoulders and face visible, no more than six months old, neutral facial expression, no designs in facial hair for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER FILE section. |
| 3. | Associate Degree in Naval Technology, major: Naval Mechanics. |
| 4. | Sea Service Certificate (12 months of sea time). |
| 5. | Seafarer’s Book (all used pages) Form P (if university intern), Form Q-1 or Q-2 (if apprentice seafarer). |
| 6. | Maritime Medical Certificate (physical and mental fitness including vision and hearing specifications), in the Maritime Medicine format available on the INEA website (must be issued by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician). |
| 7. | Payment for document issuance, to the Banesco checking account, in the name of INEA, 0134-1099-2200-0100-0780, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to the Articles: Title: 59, Item 2 and Credential: 61, Item 10 (located next to TYPE OF REQUEST). This Table will be updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and has three months of validity once it is performed”. |
| 8. | The INEA payment liquidation form will be uploaded to each user’s SEAFARER FILE, under the REMARKS tab, by the Seafarers’ Management personnel once the payment has been confirmed. |
Note: Requests before the INEA shall be made without the need to present folders or compact discs. All documents must be valid, legible, digitized in PDF format, and attached in the SINEA.
