ORIGINAL YACHT CAPTAIN LICENSE

1.	Identity Card.
2.	Updated photograph with high resolution and sharpness, blue background, formal attire (both shoulders and face visible, no glasses, no facial expressions, no shapes in the beard for men, no low-cut necklines for women, and no photomontages). It must be attached in the SEAFARER’S FILE section.
3.	First-Class Recreational Skipper License (both sides) or a Clearance Document indicating at least the rank of Lieutenant Commander within the fleet, submarine, or marine corps specialties (river boat skippers). This must be attached in the SEAFARER’S FILE section, under the CERTIFICATES tab.
4.	Sea Service Certificate (24 months of sea time as a First-Class Recreational Skipper or a certificate issued by the Floating Unit where services were rendered, endorsed by its Major Command, with a minimum of 10,000 nautical miles sailed).
5.	Recreational Seafarer’s Book (all used pages) (non-military personnel only).
6.	Maritime Medical Certificate (physical and mental fitness including vision and hearing specifications), in the Maritime Medicine format available on the INEA website (must be issued by an INEA-authorized physician or an Internist from the Public Health Network). Applicants over 40 years of age must present a cardiovascular evaluation report and an electrocardiogram (must be performed by a Cardiologist, Internist, Intensivist, or Emergency Physician).
7.	Payment for document issuance, to the Banco de Venezuela checking account, in the name of INEA, 0102-0762-25-0000003188, RIF G-20002141-1, in accordance with the amounts indicated in the SEAFARER PROCEDURES PAYMENT TABLE according to the Articles: for Original License: Article 61, Item 11; for retaking Online Exam subjects (maximum 2): Article 61, Item 15 (located next to TYPE OF REQUEST). This Table is updated at the DICOM rate every Monday at noon. “The date of the payment made must correspond with the date of the request and has three months of validity once it is performed”.
8.	The INEA payment liquidation form will be uploaded to each user’s SEAFARER FILE, under the REMARKS tab, by the Seafarers’ Management personnel once the payment has been confirmed.
